of $25 with this form will hold a seat for you.

Registration
and Cancellation Information

Deposit of $25 with registration. (Full
refund of deposit if written cancellation received on or before June
26, 2009)

Balance due on or before May 15, 2009.

No refunds after May 15, 2009, but you may transfer your reservation to a friend. Any
financial arrangements must be made between you and your friend.
Please have them register and indicate in the comments area that they
are taking over your seat on the tour.

Quilt Tours of Las
Vegas reserves the privilege of canceling the tour if an insufficient
number of registrations are received. Should Quilt Tours of Las Vegas
need to cancel the tour, a full refund will be made within 5 days of
notification.
Please note that
prices are subject to increase due to fluctuations in transportation
costs.
